If you have spoken to a member of the Hentronix team and have discussed sending your opener back for repair, please follow the instructions on this page to continue.

Please download and print, and fill out a copy of the Repair Request Sheet. This will need to be included in the package with your opener:

Please note that openers returned without the sheet may be subject to processing delays or loss.

Once you have packaged up your opener (sending all parts, including motors, does help us to diagnose problems fully), please send the opener to the address on the slip.
